How to Receive Your Rebate Checks

1. Get a contact lens fitting and eye exam.
2. Within 90 days of your exam date, purchase the required quantity of lenses from the list above. You will receive CIBA Vision and/or Vision Direct mail-in rebate forms with your contact lenses.
3. Follow the instructions on the rebate forms and mail to the addresses indicated on the forms.

Restrictions:

• Offer valid only in the United States and Puerto Rico.
• Offer limited to one (1) fitting rebate per person during the promotional period and further subject to the Terms and Conditions on the rebate form. Purchase must be made within ninety (90) days of eye exam and during Promotional Period.
• Offer not valid for groups or organizations.
• Original mail-in certificate, copy of sales receipt, and copy of valid eye exam and contact lens fitting receipt required.
• Fraudulent submissions will be disqualified and are subject to prosecution under state and federal law.
• Allow six (6) to eight (8) weeks for processing.
• Notice: if you are personally filing a claim for reimbursement from a third-party payer (e.g. insurance company, employer group, etc.) for the purchase of this product, your claim must be based upon your payment less the amount of this rebate. If your doctor is filing this claim, you must notify the doctor's office of the need to deduct this rebate amount from the purchase price used in calculating the claim.

• Rebates will be sent only to street or rural address. P.O. Box addresses will not be honored.
• Purchase must be made between May 1, 2009 and December 31, 2009. All rebate requests must be received by January 31 , 2010.
• CIBA Vision not responsible for lost, late or undelivered rebate requests or responses.
• Offer void where prohibited by law.
